- "Agreement to Mediate - Version 1" - This is a sample "kitchen Sink" version of the agreement that agencies and other parties sign before mediation.
- "Agreement to Mediate - Version 2" - This is a sample agreement to mediate involving two employees and a state agency.
- Mediator Personal Services Contract - The Department has developed a model Personal Services Contract for state agencies to use in obtaining the services of mediators and facilitators. The form may be used by any state agency.
