This guide explains how to complete and file the form for the "Claim of Violation of Crime Victims' Rights".
A crime victim may use this form to request the twelve rights that only become rights if requested. The form has two sides or pages. The first side or page may be returned directly to the district attorney typically through the Victim Assistance Program, or to the juvenile department involved with the case. The second side or page does not get returned to anyone. A victim must contact the specific agency identified in the second side/page right in order for the right(s) to go into effect. These cards are designed for law enforcement officers to give crime victims initial notice of most of their rights under the Oregon constitution. The cards also provides other information that may be useful to crime victims early in a case.
In order to offer as much contact information as possible, we divided Oregon into three regions, each with twelve counties. Each of these three regions has notification cards available in six languages. To find your region, please see the map below:

This single page, tri-fold brochure is a list of all crime victims' rights under Oregon law. Unlike the pocket/notification cards, there is one brochure for all regions, in each of the six languages. The brochure also contains some helpful phone numbers for victim services providers:

This booklet gives more information about each of the rights for crime victims than is in the single page brochure. A copy of the Victims' Rights Request Form can be found at the back of the booklet. Two versions of this booklet are available below. The first is formatted to be easily readable in PDF format. The second is arranged so that crime victims' rights practitioners can easily reproduce the materials in booklet format for distribution.
