Instructions
You have three options for submitting complaints to the Oregon Department of Justice.
- You may call the Attorney General's Consumer Hotline and request a form to be mailed to you.

- If you do not have supporting documents to send with your complaint you may file electronically using our e-mail complaint form. Fill out the requested information and click on the "Submit" button. E-mail used to convey the form input is not secure. If you are communicating sensitive information, you might want to consider sending it in by postal mail.
- If you have supporting documents, contracts, receipts etc., and want to fill out a complaint form on the computer, choose our PDF form. You can fill out the form on your computer, then print it, attach your papers (copies of your supporting documents) and mail it to:
